Close relationship with Spot-backed Antbird (Hylophylax naevius) and Dot-backed Antbird (Hylophylax punctulatus) suggested by morphology, vocalizations and nest architecture. Birds from western Colombian lowlands described as subspecies subsimilis, but considered inseparable from nominate.

The following 2 subspecies are recognised:

  • naevioides (Lafresnaye, 1847)   -  Eastern Panama (both slopes eastern from Colón and Panamá), northern and western Colombia (east to César and Santander on northern slope) and western Ecuador (south to Guayas and Chimborazo).
