Panama (eastern from eastern Bocas del Toro on Caribbean slope, from Panamá on Pacific slope), western and northern Colombia (Pacific slope, northern base of Andes east to Santander, and Magdalena Valley south to Cundinamarca) and western Ecuador (south to eastern Guayas, Chimborazo and north-western Azuay).

Subspecies
Myrmotherula surinamensis (Sibley and Monroe 1990, 1993) has been split into Guianan Streaked Antwren (Myrmotherula surinamensis), Pacific Antwren (Myrmotherula pacifica) and Amazonian Streaked Antwren (Myrmotherula multostriata) following SACC (2005).
Until recently considered conspecific with Guianan Streaked Antwren (Myrmotherula surinamensis) and Amazonian Streaked Antwren (Myrmotherula multostriata), but vocal and plumage characters indicate that they are separate species. Appears to be most closely related to Cherrie's Antwren (Myrmotherula cherriei). Genetic and morphological analyses indicate that all four species are part of a monophyletic group that also includes Pygmy Antwren (Myrmotherula brachyura), Moustached Antwren (Myrmotherula ignota), Cherrie's Antwren (Myrmotherula cherriei), Klages's Antwren (Myrmotherula klagesi), Stripe-chested Antwren (Myrmotherula longicauda), Sclater's Antwren (Myrmotherula sclateri) and Yellow-throated Antwren (Myrmotherula ambigua) (the "streaked antwren assemblage").
